Watch what you say - it doesn't take much information to drill down on a real identity. Just mentioning an institutional affiliation narrows you down to one of thousands. A program? One of a hundred or so. Your PGY year? You get the idea.

Seems to me it's kinda a toss up between intentional and not. Are the tweeting because the comment had them nervous or distracted and they missed the first time? Were they venting that of all the patients that was the one they missed on? Did they decide to punish the person with multiple sticks? Hell if I know.

For real though this new generation of med students and nursing students needs social media training in their program. The amount of coworkers who post pictures from work (taken at the nursing station or charting room, where patient info might inadvertently be in the background) is crazy. There's so many medical YouTubers/TikTok/Insta people that post stories potentially identifiable enough to be HIPAA violations.
